Medical Marijuana Sacramento Letter of recommendation




To qualify for a recommendation you must schedule an appointment and provide the following documents:
A document signed by a California licensed health provider stating your qualifying diagnosis or medical condition. The primary care physician may be a medical doctor, acupuncturist, chiropractor, physical therapist, podiatrist, or another allied healthcare professionals
This document needs to be less than 12 months old, unless you have a permanent condition such as HIV or MS. This document does not need to mention a recommendation for cannabis. Prior medicinal marijuana recommendations are not sufficient for documentation of your medical condition.
If the medical condition is chronic then you must provide proof that you are still being followed at least yearly for this condition. Medical bills, pharmacy receipts, prescription bottles, and disability documents may be helpful supporting information, but they are not medical records. Ideally what is needed are the actual treatment notes of your physician including lab reports and x-ray reports.
Examples of medical documentation include:
• a note or copy of medical records from your primary care physician which confirms in your diagnosis.
• written reports of x-rays or MRIç—´
• prescriptions that were written for the medical condition for which you would like to use medical marijuana
• recent prescription bottles for the medical condition for which you would like to use medical marijuana
• legal documents which state the medical condition for which you would like to use medical marijuana
If you have not seen a health care provider for over 12 months or have no documentation of your diagnosis, then please make an appointment with your primary provider and obtain a signed note with your qualifying diagnosis prior to making your appointment.
If you have a qualifying medical condition, but you do not have your documentation available, you may come in for the evaluation and receive a temporary recommendation which will be valid for a period of approximately one to three months depending upon your medical condition. This recommendation will allow you to immediately become a legal medical marijuana patient. You must provide our doctor with your supporting documentation within the temporary approved period of time to be upgraded to a full one-year recommendation. This upgrade is done at no additional charge.
Valid California identification includes either a California Driverç—´ License or DMV issued I.D. card. Other types of identification, such as passports, are not accepted. if you do not have a drivers license please bring another current photo ID and proof of California residency (e.g.) utility bill, bank statement, phone bill, pay stub, car registration, etc.
The I.D. must verify that you live in California. We can see any patient, living anywhere in California.
Once approved, with the appropriate identification and medical documentation, a patient will receive a recommendation to use medical marijuana. This recommendation is valid through out the State of California.
Your letter of recommendation will be signed by the physician. You will be given the original letter of recommendation. If you lose your original we will supply you with a replacement at no charge.
